What is the BCBR exam?

BCBR is the Basic Course in Biomedical Research. It is an ICMR course, run through the National Institute of Epidemiology (NIE), and it ends with a proctored online exam. For many medical professionals in India, it is compulsory.

Who has to take it

A lot of medical research in India is done by clinicians who were never trained in research methods. This course is for them: PG students, faculty, and anyone who designs studies or works with human participants.

The course is run by ICMR’s National Institute of Epidemiology, and the National Medical Commission (NMC) also has a page about it. That is why it comes up when people talk about promotions and thesis work.

Whether it is compulsory for you depends on your institution and your council, and these rules change. Check with your institution’s research cell and the two official pages: the ICMR–NIE course page and the NMC listing. The questions at the end of this page quote the official eligibility rule word for word. I have not written the exam fee here, because some third-party articles show a fee that is not on the official page, and a wrong fee is worse than no fee.

What the course covers

23 lectures, from what health research is, through study design, sampling, sample size, data collection and analysis, to research ethics and publication ethics. It is a course on research methods and biostatistics, with a lot of ethics. It is not a clinical course.

This is where many clinicians get stuck. If you are not from PSM, many of the terms will be new to you, even though the ideas are not difficult. You can see the full lecture list topic by topic.

How you get to the exam

These steps are from ICMR–NIE, not from me, and each one links to their page.

  1. Enrol on SWAYAM–NPTEL. The course has been self-paced since April 2022. As per the official course page, enrolment is open all year, and there is no deadline for enrolment or for submitting assignments.
  2. Score 50% in every lecture assignment, all 23 of them. That makes you eligible to register for the proctored exam, as per the ICMR–NIE course page.
  3. Register before the last date. ICMR–NIE posts the registration link for each exam on the course announcement page, which you can see after you enrol. Registration closes about 2-4 weeks before the exam, and late registrations are not accepted.
  4. Give the exam at a proctored centre. If you score 50% or more, you get an e-verifiable course completion certificate.

What the exam is like

100 questions in 180 minutes, and you need 50 to pass. It is online, under proctoring, at an exam centre, in the morning session from 9:00 am to 12:00 noon. ICMR–NIE holds 8 exams in 2026, and registration for each closes about 2-4 weeks before it.

Two things matter more than any study technique:

  • There is no negative marking. A wrong answer costs you nothing more than a blank. So never leave any question unanswered, always mark something.
  • You have 1.8 minutes per question. That is a lot of time. Nobody fails BCBR because of time, so there is no need to practise speed.

How people actually pass it

There is no time pressure and the pass mark is 50, so BCBR is more about recognising the question than working it out. The people who find it easy are the ones who have already practised these types of questions. The ones who struggle are usually reading the course material again and hoping it stays in their head.

So the method is simple: solve questions, read why the wrong options are wrong, and keep repeating till nothing surprises you. That is exactly what I did, and that is what BCBR Buddy is made for.

Common questions

Who is eligible for the BCBR course?

The official ICMR-NIE course page lists two groups: medical postgraduates (MD, MS, MCh, DM) admitted from the academic year 2019–20 onwards, and teachers in medical institutions. I have copied this from the official page as it is, because these rules can change. Please check the official page before you plan.

Do I have to pass the lecture assignments before I can sit the exam?

Yes. As per the official course page, you can register for the proctored exam only after you score 50% in the assignments of all 23 lectures. Many people miss this. So finish the assignments first, then register for the exam.

When is the next BCBR exam, and how do I register?

ICMR-NIE publishes a calendar for the year, and then posts the registration link for each exam on the course announcement page on SWAYAM-NPTEL, which you can see after you enrol. Registration closes about 2-4 weeks before the exam, and late registrations are not accepted. So plan from the registration last date, not the exam day. Is the BCBR exam difficult?

No, it is not difficult, just unfamiliar. The pass mark is 50, there is no negative marking, and you get 1.8 minutes per question. What troubles most clinicians is the terms. If you are not from PSM, words like confounding, sampling frame and effect modification are new, even though the ideas are simple. When I gave the exam, that was the only difficult part.

Less time than you think, if you spend it on solving questions instead of reading the lectures again. I had only 4 days. What works is solving questions, reading why the wrong options are wrong, and repeating till nothing surprises you. With a pass mark of 50 and no time pressure, recognising the question matters most.
